Care Coordination
Care Coordination is the “nut and bolts” of Rural Medical Services’ patient centered medical home strategy. We strive to improve the health of patients by working with them individually to create and meet person centered self-management goals through education and support. Care Coordinators work as a team with primary care providers and nurses to improve the patient experience of care including quality, access and reliability. As a patient, you are the heart of your Rural Medical Services Care Team.
Care Coordination Services include:
- Pre-visit planning
- Resource Connection and Coordination
- Self-management Education
- Care Planning and Management
- Supportive Services
- Patient Advocacy
- Coordination of Services between specialty providers, pharmacy, hospitals and your health care team.
- Health Education
I love Care Coordination. Probably the best job I have ever had. I feel so rewarded each day when patients ask me for advice and help. The most rewarding part is to see a smile on a patient’s face when they reach their goal or they succeed in a change in lifestyle.
