RMS uses the Patient Centered Medical Home (PCMH) Model
What is a Patient Centered Medical Home?
A “patient centered medical home” is a medical office/clinic where a team of health care professionals works together to provide evidence-based care for each patient. Rural Medical Services, Inc. provides primary care for the people of Cocke, Jefferson and surrounding counties in Tennessee. Our Family Practice physicians and Nurse Practitioners are board certified and trained as specialists in evidence-based care. RMS also offers behavioral health services to all patients. Behavioral Health Providers are located on-site at three locations to help in a whole person, patient-centered approach. Our patient-centered approach offers the combination of a traditional style family doctor assisted by a team of medical professionals with up to date technology, including a patient portal where patients can communicate with their physician/care team and check lab results via the internet.
Our Role as Your Trusted Health Care Team
- Provide a safe and healthy healthcare environment.

Your medical home should be about YOU
At RMS caring about YOU is the most important job we have. As your care team our job is to make sure you have the health care you need. Your health care team consists of a doctor or family nurse practitioner, nurses, a health educator, behavioral health, and other care team staff. Other outside health care professionals, such as pharmacists, specialists, and physical therapists may also be part of your team. Our goal is to get and keep YOU healthy by providing, finding and establishing the care that is right for YOU. Your family and caregivers are also an important part of this team. RMS is a Family Practice center that provides quality care which transitions with YOU from obstetrical, pediatric, adult and geriatric care for life long health care. If you have questions or need to be seen during business hours, you can call the front staff of your home clinic to schedule a visit, request a same day appointment, or speak with a trained nurse who can triage your call.
